The aim of the exercise program is to introduce CDMP participants to an individually tailored exercise and diet program and advice that can easily be transitioned into the home environment. The goal is to decrease health risks associated with chronic disease and improve the participant’s quality of life.
To ensure quality care and information is provided, NCACCH works in close collaboration with local GP Practices, Exercise Physiologists and Diabetes Educators/Dietitians.
Referrals to Specialists and Allied Health services are assessed and included (with a referral from a GP) to ensure participants are receiving the standard and quality of care required to assist in managing the participants chronic disease with the goal of long term self management.
This program is held three monthly in various locations across the Sunshine Coast and Gympie regions.
In collaboration with Active Living, NCACCH has recently developed an on-going maintenance exercise program, which provides the exercise program participants the opportunity to continue their exercise routines at various gym locations across the Sunshine Coast and Gympie regions.
